5+ minimum number of contracts per effective date to qualify
$150 bonus per new Metallic contract sold
$125 bonus per mew Level Funded contract sold
PLUS - $400 additional for groups over 10 enrolled

New Dental Sales Bonus
$25 per sold contract

Retention 90% Bonus
$50 per renewed contract
Combination of Level Funded, Metallic and Legacy
Transition groups are counted as renewals, not new sales

Early Renewal Bonus
