Anthem Blue Cross Optum/Monarch, Arta and Applecare Medical Groups Termination

Please be aware the below-referenced medical groups will no longer be part of the Anthem Blue Cross Priority Select HMO network beginning 1/1/2022.

  • OPTUM CARE NETWORK - ARTA HEALTH (0WL)
  • OPTUM CARE NETWORK – MONARCH (0UW)
  • OPTUM CARE NETWORK APPLECARE SELECT (ZUD)

Anthem is making every effort to ensure that members continue to have access to their current primary care physicians and specialty providers through alternate medical group affiliations.

Members have been informed that their employers might offer alternative Anthem Small Group plans on the California Care HMO network and Select HMO network which will continue to include the above-referenced medical groups.

Next steps:

  • Members affected by this change will be assigned to alternate participating medical groups effective January 1, 2022.
  • Anthem will mail new ID cards reflecting this change no later than five days before January 1, 2022.
  • Members should familiarize themselves with their new medical group, as the hospitals and specialists available may be different from those offered by their current medical groups.
  • Members can also use the Find a Doctor feature at www.anthem.com/ca to see what providers are available to select a new primary care physician.
  • All plan benefits remain unchanged.

Members should contact an Anthem Blue Cross customer service representative using the toll-free customer service number, 1-844-971-0117, OR the toll-free member services number on their current ID card if they have concerns regarding their care.
