Choice Builder® announced a new Dental HMO (DHMO) plan in the Bronze metal tier, which will be available on May 1, 2019. Employees will have three DHMO (Bronze, Silver, or Gold) plans to choose from. Rates start at $11.32 for employee-only coverage. We have the benefit details available for you to review.
There will be no changes to the current employer application; it’s valid beyond May 1, 2019 effective dates. However, two new employee forms are required for May 1, 2019 effective dates and beyond:
