Florida Blue is expanding its virtual care coaching program, conducted by Livongo, for select commercial members with low-acuity (low-severity) diabetes and hypertension. Members who qualify will begin receiving emails from Livongo over a 12-week period, inviting them to participate in one of the programs.
This is an expansion of their existing program, which was initiated in April 2021. The expansion will now include Diabetes Complete (diabetes + hypertension + weight management) and Hypertension Complete (hypertension + weight management) for low-acuity members with an "anchor condition" of diabetes or hypertension.
Members in the existing diabetes management program will receive the new features and benefits available through the Diabetes Complete Program:
- Blood pressure monitor that maintains a private log of readings and provides real-time personalized tips after each reading
- Cellular-enabled digital scale and weight management support
In addition, Florida Blue has identified 100,000 members with hypertension who will now be eligible for the Hypertension Complete Program. They will be targeted with an initial 12-week recruitment email campaign, managed by Livongo. Florida Blue will also be adding newly identified members to both programs each month.
Because the program is limited to a select population, other participants are not eligible at this time. Sanitas-assigned and myBlue Connected Care product members are also excluded, as Sanitas offers its own diabetes management program. Members with hypertension and/or diabetes who are not eligible for this program may call their local Florida Blue Center at (877) 352-5830 for information about other options to meet their needs.
