Average state-wide rate action for Q4 2025
- HMO 3.5%
- PPO 3.4%
*Average rate change across all regions, plans and metal tiers.
Dental/Vision: no rate or plan changes at this time.
Health Net's Enhanced Choice Simplified Underwriting promotion has been extended through Q4:
- 4+ enrolling employees, mix & match any plans from the HMO and PPO networks.
- NO DE9C, payroll, prior carrier bill, or ownership documents are required.
- 4-9 enrolling employees, 25% participation required.
- Employees enrolled on another carrier through the same employer will not count against participation.
- 10+ enrolling employees, no participation requirements and no waivers -when not paired with Life or Employer paid Dental/Vision.
