Effective May 31, 2022, in Santa Cruz County, Dignity Health Medical Group - Dominican (DHMGD) will no longer participate in the Sutter Health Plus provider network. However, the Dignity Health Dominican Hospital will continue as an in-network hospital.
Sutter Health Plus will help members with Dignity primary care physicians (PCPs) establish care with Palo Alto Foundation Medical Group (PAFMG) and Sutter-affiliated Palo Alto Medical Foundation (PAMF). PAFMG is a nationally recognized and respected medical group with more than 1,600 clinicians in the Bay Area, including 56 PCPs in Santa Cruz County accepting new patients.
As of May 31, 2022, the Sutter Health Plus network in Santa Cruz County will include:
- 167 PAFMG primary care and specialty doctors and clinicians
- Virtual-first primary care through Tera: PCP Dr. Jocelyn Tseng, M.D. and her care team with a physical location in the community
- Same-day access to care for simple, everyday health needs through Sutter Walk-In Care
- Access to three network urgent care locations for timely care for unforeseen illnesses or injuries requiring immediate attention
- Emergency, inpatient, and outpatient care at Dignity Health Dominican Hospital and Watsonville Community Hospital
- Comprehensive maternity care and surgical services at Sutter Maternity Surgery Center
The Sutter Electronic Health Record connects PAFMG and other Sutter-affiliated providers, allowing them to share critical health information to better coordinate care and improve health outcomes. Members will also have access to My Health Online allowing them to message clinicians, schedule appointments, see their PCP and some specialists through video visits, renew prescriptions, view test results and health information, and manage their health online.
Sutter Health Plus is notifying your impacted employer clients and members about this transition. Sutter Health Plus will also assist members undergoing a course of treatment in transitioning care to in-network providers.
